Before you apply to a Caribbean medical school from Pakistan, check PM&DC recognition, because foreign medical degrees are not automatically recognised in Pakistan. PM&DC says graduates of unrecognised institutions will not get provisional registration. This guide covers the PM&DC rules, accreditation, and visas for Cayman, Grenada and Saint Kitts and Nevis.
PM&DC rules to check before you apply
PM&DC’s notice of 22 October 2025 says graduates of foreign institutions it does not recognise “shall not be issued provisional registration.” Sitting its National Registration Examination depends on the school being recognised by its own regulator and accredited or listed by ECFMG. Its policy for students entering foreign medical schools also sets these conditions:
- FSc Pre-Medical with at least 60%, or equivalent SAT-II or foreign MCAT scores, and at least 50% in MDCAT conducted in Pakistan.
- A school on the ECFMG approved list, an allopathic degree, and at least 6,200 hours over five years with 80% attendance.
- Six months of local-language learning before leaving if the course is not taught in English.
- The free PM&DC Student Registration Certificate (srf.pmdc.pk) before you take admission, and a multiple-entry visa for the whole programme.
- Passing the exam. Non-compliance makes you ineligible.
A PM&DC notice of 14 May 2026 repeats the MDCAT, FSc and registration rules. Another notice says its list of foreign institutions was valid only until the 2024-25 admissions. We could not read any PM&DC list of recognised Caribbean schools, and the policy does not mention the Caribbean. Never treat a school as “PM&DC recognised” until you have checked pmdc.pk.

ECFMG requires, from 2024, a medical school accredited by an agency recognised by WFME. WFME announced renewal of ACCM’s recognition on 28 August 2026, with no expiry stated. The ACCM claims above come from the schools themselves, and we did not open each school’s ECFMG directory entry, so we cannot confirm USMLE eligibility school by school.
Student visas by island

Cayman student visa holders may not take gainful work other than unpaid student work, and Grenada requires Ministry of Labour permission. Cayman says graduates may apply for a work permit afterwards, not guaranteed. Cayman allows a spouse and children to accompany a student who proves financial support.
English requirements and entry
SGU asks applicants from countries not counted as English-speaking for IELTS Academic 7.0 overall and in each band, or Duolingo 130, with a waiver possible for a degree from an English-speaking country. Non-North American applicants do not need the MCAT. SGU offers 5, 6 and 7-year tracks depending on your qualifications, and its pages do not mention the Pakistani FSc or HSSC, so we ask SGU directly.
We did not read English scores, entry routes or tuition for St. Matthew’s, the Medical University of the Americas, UMHS or Windsor.
Costs, dates and how we help
SGU’s MD starts on 18 January, 7 April and 16 August 2027. St. Matthew’s starts in January, May and September, with the first five semesters on Grand Cayman and a USD 500 deposit applied to the first semester. We found no tuition figures, scholarships or insurance rules we can quote.
